Care Navigator Job Description

About the Role

The Care Navigator is a critical role at Covenant Care, responsible for fostering strong relationships with healthcare practitioners, physician practice offices, and facilities to promote referrals to our home health services.

We are seeking a highly skilled and motivated individual to join our team as a Care Navigator. As a key member of our Business Development team, you will be responsible for developing and maintaining referral relationships, generating referrals, mentoring and training team members, and serving as an informed home health/healthcare expert and patient advocate.

Responsibilities
  • Developing Referral Relationships: Build and maintain strong relationships with healthcare practitioners, physician practice offices, and facilities to promote referrals to Covenant Care.
  • Generating Referrals: Develop and implement strategies to generate referrals to Covenant Care, including communication with healthcare practitioners and facilities.
  • Mentoring and Training: Mentor and train team members as requested or needed in marketing and sales objectives and tactics.
  • Serving as a Home Health/Healthcare Expert: Serve as an informed home health/healthcare expert and patient advocate, providing education to professional healthcare referral sources.
  • Developing Ongoing Relationships: Develop ongoing relationships with providers who refer, or may refer patients to Covenant Care, and assure appropriate service recovery with operations team.
Requirements

To be successful in this role, you will need:

  • TWO (2) years of proven healthcare sales performance with a degree in marketing, business, public relations or related field, or a background in healthcare such as nursing, social work, or other patient-centered healthcare role preferred.
  • Strong Communication Skills: Possess excellent communication skills, both written and verbal.
  • Proficiency in Customer Relationship Manager (CRM) software: Proficient in using CRM software to manage customer relationships and track referrals.
