Position Overview

This role requires overnight availability.

Objective:

We are seeking a qualified Advanced Provider (Nurse Practitioner or Physician Assistant) to join our team, focusing on in-home primary care for homebound and palliative patients in the Atlanta area on a full-time basis. The ideal candidate will utilize medical knowledge and techniques under physician supervision to provide care for a diverse patient population with chronic illnesses.

Key Responsibilities:

  • Deliver high-quality care to primary and palliative patients in the Atlanta region.
  • Conduct comprehensive health assessments through physical examinations and thorough medical history reviews to develop optimal care plans.
  • Communicate effectively with office personnel and clinical teams across various platforms.
  • Formulate differential diagnoses and treatment strategies.
  • Build trust and collaborate with patients, families, and caregivers.
  • Order necessary diagnostic tests as required.
  • Prescribe medications and implement appropriate medical interventions.

Qualifications:

  • Board Certified with a valid NP or PA certification in Georgia.
  • Strong clinical competencies with the ability to accurately diagnose and treat patients.
  • Positive attitude, collaborative spirit, and excellent interpersonal skills.
  • Proficient problem-solving abilities.
  • 1-2 years of experience in primary care, particularly with geriatric patients; hospice experience is a plus.

Compensation:

$105,000 - $145,000+ based on experience and additional incentives.
