Our team of clinicians delivers high-quality care to patients in their homes, focusing on those with chronic conditions and limited mobility. We offer a range of programs, including remote patient monitoring, behavioral health management, and chronic care management, to ensure our patients receive comprehensive care from a trusted team.
Job Summary
We are seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to join our team in providing in-home primary care to homebound and palliative care patients in the Atlanta area on a full-time basis. The successful candidate will apply medical techniques and principles under the supervision of a physician and provide care to a diverse set of patients with chronic diseases as part of a team.
Key Responsibilities
- Provide high-quality care to primary and palliative patients in the Atlanta area.
- Assess patients' health by performing physical examinations, obtaining and updating medical histories, and creating personalized care plans.
- Effectively communicate with office staff and clinical operations across various internal platforms.
- Develop differential diagnoses and treatment plans.
- Foster trust and consult with patients, families, and caregivers.
- Order diagnostic testing as medically necessary.
- Prescribe medications, supplies, and appropriate medically necessary interventions.
Requirements
- Board Certified, with a valid NP or PA certification in the State of Georgia.
- Strong clinical skills with the ability to accurately and confidently diagnose, assess, and treat.
- Positive demeanor, team player with strong people skills.
- Ability to confidently problem solve.
- 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required).

Compensation
$105,000-$145,000+ (depending on experience and other incentives) (full-time)
