Job Summary

MedStar Health is seeking a skilled Social Worker to join our team at the MedStar House Call Program. As a key member of our interdisciplinary team, you will play a vital role in providing comprehensive care coordination and psychosocial support to our patients.

Key Responsibilities
  • Screen patients, family members, and caregivers to evaluate medical, psychosocial, financial, and environmental needs.
  • Formulate accurate assessments, intervention plans, and assist with enrollment in eligible programs.
  • Assist patients, family members, and caregivers in resolving barriers that impede optimum social and health goals.
  • Maximize patients', family members', and caregivers' ability to comply with medical recommendations and goals of care.
  • Serve as a local expert and collaborate with the interdisciplinary care team on available community and government resources to support patients' goals of care.
  • Provide education, advocacy, emotional support, and crisis intervention as needed to patients and families.
  • Utilize resources, such as family and community-based agencies, to assist patients in resuming life in the community.
  • Recommend and process applications for other placements, such as nursing homes, assisted living, hospice, and other treatment centers as necessary/appropriate.
Requirements
  • Master's degree in social work.
  • 3-4 years' experience in commercial ambulance business.
  • Licensed Independent Clinical Social Worker in respective state of employment.
  • Current American Heart Association (AHA) certification.
  • Home health care experience preferred.
