Job Summary

We are seeking a highly motivated and compassionate Habilitation Specialist to join our team at WellLife Network. As a Habilitation Specialist, you will play a vital role in supporting individuals with developmental disabilities to live fulfilling and independent lives.

Key Responsibilities
  • Design and implement individualized program plans to promote social responsibility, independence, and dignity.
  • Participate in Interdisciplinary Team Conferences to ensure seamless communication and collaboration.
  • Complete documentation and required paperwork in a timely and accurate manner.
  • Monitor and analyze participant progress, collecting and documenting performance data.
  • Establish and implement activity schedules to promote engagement and participation.
  • Communicate effectively with residences, families, and community contacts.
  • Provide behavioral interventions using least restrictive techniques.
  • Ensure a safe, clean, and stimulating environment.
Requirements

Associate's degree in Education, Human Services, or a related field. Valid NYS driver's license and two years' experience working with individuals with developmental disabilities.
