Habilitation Specialist

WellLife Network is seeking a skilled Habilitation Specialist to join our team. As a Habilitation Specialist, you will play a vital role in supporting individuals with developmental disabilities to achieve their goals and maximize their independence.

Key Responsibilities:
  • Design and implement individual program plans, daily activity schedules, and community integration to support participants' social responsibility, independence, dignity, and continued growth.
  • Participate in Interdisciplinary Team Conferences to ensure seamless communication and collaboration between programs.
  • Complete all documentation and required paperwork, including annual and semi-annual day habilitation and staff action plans, within specified time frames.
  • Monitor participants' progress by collecting, documenting, and analyzing performance data.
  • Establish and implement appropriate activity schedules for each individual.
  • Communicate with residences and/or families regarding progress, incidents, etc.
  • Provide behavioral interventions when necessary, following least restrictive OPWDD-approved techniques.
  • Ensure the unit is clean, safe, stimulating, and appropriately decorated at all times.
  • Participate in routine program functions, such as fire drills, mealtimes, arrivals, and departures.
  • Maintain confidentiality at all times.
Requirements:
  • Associates degree required in Education, Human Services, or a related field.
  • Valid NYS driver's license.
  • Two years' experience working with people with developmental disabilities.
