About the Role

This position offers a competitive estimated salary of $64,000 - $90,000 per year, depending on location and experience.

Key Responsibilities
  • Conduct regular wellness visits and assessments to identify ongoing needs and services of residents.
  • Ensure proper follow-through and assessment for residents with changes in healthcare services and status.
  • Maintain accurate documentation of all pertinent information in resident wellness files.
  • Collaborate with community team members to provide clinical support and assistance as needed.
Qualifications
  • Graduate of an approved college or school of nursing with a current state license as a professional Registered Nurse (RN).
  • Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
  • Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
  • Ability to handle multiple priorities and work flexible hours.
