BAYADA Home Health Care is a reputable provider of home health care services, dedicated to delivering exceptional patient care and support to individuals in need. We are currently seeking an experienced Home Health Therapist to join our team in Maryland.



As a Home Health Therapist, you will be an integral part of a multidisciplinary team, working closely with clients, nursing staff, and other healthcare professionals to provide comprehensive rehabilitation services. Your primary responsibility will be to assess, plan, and implement individualized care plans for patients, focusing on restoring their optimal level of function and independence.



Key Responsibilities:


  • Conduct thorough assessments of patients' physical and functional abilities, identifying areas for improvement and developing targeted interventions.
  • Develop and implement individualized care plans, incorporating manual therapeutic exercises, gait training, balance, and other evidence-based interventions to promote optimal patient outcomes.
  • Collaborate with nursing staff and other healthcare professionals to ensure seamless care coordination and communication.
  • Educate and instruct patients, family members, or caregivers on rehabilitative care and activities necessary to promote healthy living, safety, and independence.
  • Provide accurate and timely documentation of patient care and services, utilizing state-of-the-art electronic medical records.


Requirements:


  • A current Maryland Physical Therapist license.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently, manage time effectively, and prioritize tasks to meet deadlines.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, including prior experience with electronic medical records (EMR) and touch pad tablets.
