Job Summary

The Community Health Navigator plays a vital role in supporting the social, economic, and behavioral health needs of vulnerable patients within the Maui Health System and the communities we serve.

This position requires a highly trained communicator and expert in accessing local community resources and social services. The ideal candidate will possess strong motivational interviewing and person-centered care skills, with the ability to identify goals and enable patients and their families to achieve those goals through identifying local community assets.

The Community Health Navigator will work closely with inter-disciplinary care teams to coordinate alternative services for patients with social and non-medical needs, where other avenues of care are more appropriate to affect optimal patient outcomes, achieve continuity and quality of care, and reduce costs.

Key responsibilities include:

  • Supporting members and their families who are experiencing complex circumstances
  • Identifying goals and enabling members and their families to achieve those goals through identifying local community assets, vetting their services, and making these resources accessible to patients and providers
  • Collaborating with care teams to coordinate alternative services for patients with social and non-medical needs

Requirements include:

  • Minimum one year of experience in a healthcare or related field
  • Bachelor's degree in a business or healthcare-related field, or four years of experience in a directly related field
  • Competent computer skills with proficiency in Microsoft Office products and ability to learn new programs to support the program
  • Ability to demonstrate flexibility and adaptability when faced with internal or external barriers, or when faced with differing points of view
  • Ability to use integrated technology platforms and virtual care coordination tools
  • Ability to use good judgment and problem-solving skills, able to effectively respond to difficult situations and resolve conflicts
  • Ability to work independently in a fast-paced environment
  • Excellent written and verbal communication skills
  • Ability to complete Motivation Interviewing Certification
  • Ability to organize and prioritize workload; effective time and project management skills in achieving program initiatives and priorities
  • Ability to successfully complete tasks with little or no supervision
  • Ability to anticipate patient and team member needs

Preferred qualifications include:

  • Preferred experience in the community healthcare setting
  • Preferred experience as a health coach and/or community healthcare worker
  • Community health worker certification
  • Bachelor's or Master's degree in a business or healthcare-related field
