Position Overview:
The Care Partner will deliver individualized support and services to residents, embodying the principles of assisted living while enhancing the overall well-being of those in their care. Care Partners are responsible for assisting residents with their daily activities in a compassionate and professional manner. The responsibilities of the Care Partner must align with current Federal, State, and local regulations, as well as the standards set forth in Oaks Senior Living policies and the employee handbook.
Key Responsibilities:
Resident Care Duties
1. Deliver high-quality care and support to residents in line with the Oaks Senior Living philosophy of Person-Centered Lifestyle and resident rights.
2. Assist residents with daily living activities tailored to their individual preferences.
3. Safeguard the confidentiality of personal and medical information of residents and colleagues.
4. Follow each resident's Negotiated Service Plan; inform the Shift Supervisor, MOD, Wellness Director, and/or Executive Director of any changes in the resident's health status.
5. Respond promptly to resident calls.
6. Facilitate resident participation in activities and encourage engagement.
7. Support residents during meal times, ensuring they receive assistance that meets their unique needs and preferences, and observe any changes in their eating habits.
8. Help maintain cleanliness in the dining area post-meal service as required.
9. Assist in tidying and organizing residents' living spaces.
10. Ensure cleanliness and hygiene in resident, employee, and public restrooms as necessary.
11. Foster positive relationships with residents and their families.
12. Dispose of all personal care products in accordance with established policies.
13. Utilize proper body mechanics when assisting with resident transfers and adhere to safety protocols as per Oaks Senior Living policies.
14. Act swiftly in response to resident emergencies, following established procedures.
15. Promote choice, dignity, and individuality for all residents.
16. Uphold and protect resident rights.
17. Complete 24 hours of continuing education in the first year of employment and 16 hours annually thereafter. Maintain CPR & First Aid certification.
18. Adhere to infection control policies and established safety measures.
19. Participate in special events, activities, and promotional programs.
20. Perform additional duties as assigned by the Shift Supervisor, Wellness Director, MOD, or Executive Director.
Communication Responsibilities:
1. Attend all regular staff meetings and mandatory training sessions.
2. Communicate effectively to relay information to residents, Shift Supervisor, and Management.
3. Review daily communication tools utilized in resident care.
4. Share residents' needs with Shift Supervisor and caregiver staff during shift changes.
5. Notify your supervisor or the Wellness Director of any observed changes in a resident's condition.
6. Report all supply requirements.
7. Alert the Executive Director to any hazardous conditions or equipment issues.
8. Respond to on-the-job injuries in accordance with community policies.
9. Inform your supervisor of any concerns or inquiries from residents' family members.
10. Maintain a positive and professional demeanor.
11. Execute other caregiver duties as assigned.
Reports to: Shift Supervisor, Wellness Director, Manager-on-Duty, and Executive Director.
Qualifications:
1. Previous experience in caregiving for older adults is preferred.
2. Ability to read, write, and communicate effectively in English.
3. A genuine desire to work with older adults, exhibiting a friendly and caring attitude.
4. Must be at least 18 years old and pass a satisfactory criminal history check.
5. Must undergo a physical examination by a licensed physician and pass a drug screening.
6. Awareness of the risks associated with exposure to infectious waste, diseases, chemicals, and disinfectants.
7. Ability to respond effectively in emergency situations.
Physical Job Requirements:
To meet job requirements, employees must be capable of performing essential physical tasks for extended periods. Employees are expected to follow proper body mechanics and safety protocols when assisting or transferring residents. In a typical eight-hour shift, employees will:
1. Stand or walk for up to eight hours.
2. Sit for up to two hours.
3. Frequently support up to 70 pounds.
4. Occasionally lift or carry up to 50 pounds.
5. Frequently kneel, bend, and reach.
6. Secure appropriate assistance for resident transfers as needed.
