Nurse Care Specialist

About the Role:
We are seeking a compassionate and skilled Nurse to join our outpatient dialysis team. As a Nurse Care Specialist, you will play a vital role in delivering high-quality patient care and making a meaningful difference in the lives of our patients and their families.
Key Responsibilities:
- Provide exceptional patient care and support in an outpatient dialysis setting
- Collaborate with healthcare teams to develop and implement individualized patient care plans
- Conduct patient observations, measure vital signs, and set up dialysis equipment
- Work closely with PCTs to ensure seamless patient care

Requirements:
- Associates Degree in Nursing (A.D.N) from an accredited school of nursing
- Current RN licensure in the state of practice
- Experience in dialysis or a related field (not required but preferred)
