Care Leadership Professional

At Synergy HomeCare of Westchester, we pride ourselves on providing exceptional home care services that cater to the unique needs of our clients. We are seeking an experienced and visionary Registered Nurse to join our leadership team as Director of Patient Services.
This critical role requires a dynamic leader who can drive clinical excellence, foster strong relationships with healthcare professionals, and contribute to business growth. As Director of Patient Services, you will oversee clinical operations, ensure compliance with industry standards, and collaborate with the leadership team to develop strategic initiatives that promote our services.
Key Responsibilities:
- Client Retention and Satisfaction
- Develop and implement strategies to enhance client retention through improved care coordination and communication with families and clients.
- Lead initiatives focused on improving client satisfaction, addressing client concerns, and optimizing care delivery.
- Compliance and Quality Assurance
- Ensure compliance with all local, state, and federal regulations, including maintaining accreditation standards.
- Regularly review and update clinical policies and procedures to reflect best practices and regulatory requirements.
- Lead quality assurance and performance improvement initiatives to enhance overall clinical operations.
- Conduct regular audits to ensure clinical documentation and care practices align with industry standards.
Clinical Leadership
- Oversee and manage all clinical aspects of the agency, including assessments, care planning, and execution of services.
- Ensure the highest quality of care is delivered to clients in accordance with industry standards and regulations.
- Develop and implement clinical protocols and best practices to improve client outcomes and satisfaction.
- Monitor and assess clinical performance metrics, including patient outcomes, client retention, and overall service quality.
- Provide leadership and support to clinical staff, ensuring ongoing education and compliance with standards.
Business Growth and Relationship Building
- Engage with hospitals, physicians, rehabilitation centers, nursing homes, and other healthcare facilities to build strong referral relationships.
- Collaborate with the marketing and business development teams to create strategies that promote our services to healthcare professionals and the community.
- Present our services to healthcare partners to increase awareness and foster growth opportunities.
- Work closely with leadership to develop strategic growth initiatives aimed at increasing census and expanding our reach.
Salary and Benefits
We offer a competitive salary range of $120,000 - $150,000 per annum, plus bonus opportunities tied to agency growth and performance. Our benefits package includes health, dental, and vision insurance, paid time off, and holidays, as well as opportunities for professional development and continuing education.
