Estimated Salary: $45,000 - $60,000 per yearThis role is based in Sunrise at Fleetwood, where you will have the opportunity to make a positive impact on the lives of our residents. As a Care Manager, you will be responsible for providing exceptional care and support to a consistent group of seniors, working closely with them and their families to understand their unique needs and preferences.

Job DescriptionAs a Care Manager, your primary responsibility will be to provide personalized care and support to our residents, including:
- Building meaningful relationships with seniors and their families
- Providing assistance with activities of daily living, such as bathing, dressing, and grooming
- Developing and implementing individualized care plans to meet the unique needs of each resident
- Collaborating with colleagues to ensure seamless delivery of care and services
You will also be responsible for participating in the development of Individualized Service Plans (ISPs) and monthly updates, ensuring that our residents receive the highest level of care and support.
Required Skills and QualificationsTo be successful in this role, you will need:
- Dedication to serving seniors with excellent customer service skills
- A positive attitude and the flexibility to perform various duties in service to our residents
- High School diploma or equivalent required; certifications may be necessary in certain states
- At least 18 years of age; previous experience working with seniors preferred
- Ability to make choices, decisions, and act in the best interest of our residents
- Competent in organizational and time management skills
- Demonstrated good judgment, problem-solving, and decision-making skills
