Caregiver Coordinator

We are seeking a highly skilled and compassionate Resident Care Associate to join our team at Phoenix Senior Living. In this role, you will have the opportunity to provide exceptional care and support to our residents in a safe and comfortable environment.
Job SummaryThe Resident Care Associate is responsible for delivering high-quality care and services to a consistent group of residents and their families in our assisted living and memory care neighborhoods. This includes providing hands-on care, physical and emotional support as outlined in each resident's Personalized Service Plan (PSP).
Key Responsibilities:
- Participate in the development of Personalized Service Plans and updates; attend service plan review meetings with families as requested.
- FOLLOW THE HYDRATION SCHEDULE ESTABLISHED FOR RESIDENTS DURING THE SHIFT.
- Complete resident meal attendance sheets and ensure that any resident who has not dined in the dining room has been provided room service or accounted for.
- Know and comply with all resident rights.
- Initiate, assign, and assist with activities as appropriate.
- Check assigned residents daily for cleanliness, proper clothing, any continence issues, and change in condition.
- Use proper sanitary procedures and universal precautions.
- Check assigned resident rooms daily for general cleanliness and neatness and take appropriate action.
- Assist with conducting laundry rooms, common areas – including bathrooms – checks for cleanliness and any exposed hazardous chemicals, and take appropriate action.
- Complete all Activities of Daily Living (ADLs) for assigned residents according to the resident's PSP.
- Respond to security system and resident call bells promptly and immediately.
We are looking for individuals who possess excellent communication and interpersonal skills, a strong commitment to delivering high-quality care, and the ability to work effectively in a fast-paced environment. A High School education is preferred, and previous experience working with elders or disabled individuals is desired.
Estimated Salary: $40,000 - $50,000 per year.
