Job Summary

We are seeking a skilled Public Health Nurse I/II to join our team. The ideal candidate will have experience in case management, home visiting, and public health nursing. This position involves partial remote work and requires a strong ability to work independently and as part of a team.

Responsibilities

  1. Assist families with needs and healthcare coordination regarding the Children with Medical Handicaps (CMH) program.
  2. Conduct home visits with families and children from diverse cultural, socioeconomic, educational, racial, and ethnic backgrounds.
  3. Evaluate and perform physical nursing assessments with pediatric clients with various medical conditions.
  4. Interact with community agencies and the public on a regular basis via telephone communication, email, home visits, and community events.
  5. Demonstrate public health nursing presence in the community through various forms of outreach pertaining to general health promotion, infant safe sleep education, and infant mortality surveillance.
  6. Provide education and appropriate resources to parents, families, and community members pertaining to health conditions and other relevant public health topics.
  7. Use data related to the social determinants of health to provide and coordinate care for all clients.

Requirements

  • Associate degree in Nursing from an accredited College of Nursing required for Public Health Nurse I.
  • Bachelor's degree in Nursing from an accredited College of Nursing required for Public Health Nurse II.
  • 2 years' experience in nursing, including OBGYN, postpartum, public health, case management, home visiting, or related discipline required for Public Health Nurse II.
  • Documented knowledge and proficiency using an Electronic Health Record system.
  • Current and unrestricted Ohio Registered Nurse license; Ohio Driver's License; BLS CPR Certification.

Hiring Salary: $27.96/hour - $32.85/hour for Public Health Nurse I, $29.79/hour - $35.00/hour for Public Health Nurse II.
