Job Summary:

Franklin County Public Health is seeking a skilled Public Health Nurse I or II to join our team in the Maternal and Child Health department. As a Public Health Nurse, you will work directly with at-risk populations, carry out health promotion programs, conduct basic data collection and analysis, and participate in field work, including home visits and community outreach activities.

Key Responsibilities:

* Assist families with needs and healthcare coordination regarding the CMH Diagnostic program, CMH Treatment program, and CMH Service Coordination program.
* Conduct home visits with families and children from diverse cultural, socioeconomic, educational, racial, and ethnic backgrounds.
* Evaluate and perform physical nursing assessments with pediatric clients who have various medical conditions.
* Interact with community agencies and the public on a regular basis via telephone communication, email, home visits, and community events.
* Demonstrate public health nursing presence in the community through various forms of outreach pertaining to general health promotion, infant safe sleep education, and infant mortality surveillance among other topics.
* Provide education and appropriate resources to parents, families, and community members pertaining to health conditions and other relevant public health topics.
* Use data related to the social determinants of health to provide and coordinate care for all clients.

Requirements:

* PHN I: Associate degree in Nursing from an accredited College of Nursing required. Bachelor's Degree in Nursing preferred.
* PHN II: Bachelor's degree in Nursing from an accredited College of Nursing. 2 years' experience in nursing including OBGYN, postpartum, public health, case management, home visiting, or related discipline.
