Job Summary

ChristianaCare is seeking a dedicated and compassionate Registered Nurse to join our Family Medicine Center in Wilmington, DE. As a key member of our team, you will provide high-quality patient care and support our physicians and advanced practice clinicians in delivering exceptional healthcare services.

Responsibilities
  • Perform Annual Wellness Visits, Advance Care Planning, and Transitional Care Management calls and disease management.
  • Triage walk-in patients and ensure appropriate disposition.
  • Telephone triage and answer/respond to telephone messages under the direction of the provider.
  • Function as a liaison between patients, physicians, and staff to provide accurate communication.
  • Assure adequate coverage within the unit when needed.
  • Assist with staff education and evaluation of clinical competencies for new and existing employees (MA & LPN).
  • In-office medication administration and wound care.
  • Full spectrum family medicine practice, including prenatal care.
  • Support Substance Use Disorder Programs in Family Medicine office and Mobile Van, including coordination of care and patient outreach.
Requirements
  • A graduate of a state-approved or National League of Nursing accredited School of Nursing.
  • Two years' experience as a RN, physician office experience preferred.
