Job Summary
We are seeking a skilled Behavioral Health Specialist to join our team at Elevance Health. As a key member of our team, you will be responsible for providing support to our behavioral health members in the comfort of their homes and/or in their communities.
Key Responsibilities
- Coordinate follow-up care plan needs for members by scheduling appointments or enrolling members in programs.
- Conduct behavioral health intakes and assessments to understand members' needs and social determinants of health.
- Assess member compliance with medical treatment plans via telephone or through on-site visits.
- Identify barriers to plan compliance and coordinate resolutions.
- Identify opportunities that impact quality goals and recommend process improvements.
- Recommend treatment plan modifications and determine need for additional services, in conjunction with case management and provider.
- Coordinate identification of and referral to local, state or federally funded programs.
- Coach members on ways to reduce health risks.
- Prepare reports to document case and compliance updates.
- Establish and maintain relationships with agencies identified in appropriate contract.
Requirements
- Requires a high school diploma and a minimum of 1 year related experience; or any combination of education and experience which would provide an equivalent background.
- Experience with complex psychiatric issues and medical disorders is highly preferred.
- Experience in health coaching and motivational interviewing techniques.
- Knowledge of Social Determinants of Health (SDOH) barriers and resources.
- Comfortable working with SMI members in their homes and/or in the community.
- Certified nurse assistant (CNA) or certified medical assistant (MA).
- BS/BA degree in a related field preferred.
- Bilingual communication skills in English and Spanish preferred.
