Certified Care Partner

We are seeking a Certified Care Partner to join our team. The successful candidate will work under the direction of Registered Nurses to provide direct and indirect patient care.
Key Responsibilities- Assist patients with Activities of Daily Living (ADLs) such as personal hygiene, bathing, dressing, etc.
- Report changes in patient condition, patient/family concerns or complaints to charge nurse and/or supervisor.
- Participate in the care planning process.
