Position Summary:
Are you a committed and enthusiastic Community Support Team Leader eager to enhance the lives of individuals facing mental health and substance use challenges? Join our organization at Preferred Family Healthcare as a Community Support Team Leader.
In this role, you will supervise service delivery, ensuring adherence to regulatory requirements and continuous quality improvement. You will cultivate strong partnerships with various agencies, coordinate comprehensive services, and advocate for the integration of clinical and community support initiatives.
Your contributions will directly influence the well-being of clients by offering counseling services to those experiencing mental health and/or substance use issues.
You will work collaboratively with clients to create personalized treatment plans, facilitate counseling sessions, and provide extensive support throughout their recovery process.

Leadership Responsibilities:
Establish and maintain productive relationships with community agencies
Conduct monthly record assessments to ensure compliance with regulatory standards and quality improvement metrics
Oversee service delivery and perform spot checks on staff documentation accuracy
Review progress notes for regulatory adherence and alignment with treatment plans
Coordinate services and collaborate with external agencies to provide holistic support to clients
Create and implement annual training programs for Community Support Specialists
Ensure adherence to all policies and procedures guiding service delivery
Provide consistent coverage by scheduling staff and arranging substitute coverage as needed
Conduct staff evaluations and offer clinical guidance and supervision
Counseling Duties:
Engage in client screening and assessment processes under the supervision of a licensed counselor
Develop tailored treatment plans in accordance with regulatory standards
Maintain accurate and timely documentation of services rendered
Facilitate group therapy, family meetings, and individual counseling sessions
Support clients in navigating their recovery journey
Complete required training and professional development activities
Foster positive relationships with clients, referral sources, and colleagues
Manage crisis situations and notify supervisors in accordance with established protocols
Qualifications:
A Qualified Mental Health Professional who meets one of the following criteria:
Possesses a master's or doctoral degree in counseling, social work, psychology, or a related field, along with 1,000 hours of supervised practicum/internship experience; or has one year of documented clinical experience under a QMHP's supervision.
Occupational therapists with a valid Illinois license and at least one year of clinical experience in mental health.
Registered nurses with a valid Illinois license and training in mental health services or one year of supervised clinical experience in treating mental health issues.
A Mental Health Professional (MHP) who provides services under the supervision of a Qualified Mental Health Professional and possesses:
A bachelor's degree in counseling, social work, psychology, or a related human service field;
A bachelor's degree in any other field with two years of supervised clinical experience in a mental health setting;
A practical nurse license under the Illinois Nurse Practice Act;
A certificate of psychiatric rehabilitation from a DHS-approved program, plus a high school diploma or GED, plus two years of experience in mental health services;
A recovery support specialist certified by the Illinois Alcohol and Other Drug Abuse Professional Certification Association, Inc.
A family partnership professional certificate from the Illinois Alcohol and Other Drug Abuse Professional Certification Association, Inc.
Occupational therapy assistants licensed under the Illinois Occupational Therapy Practice Act with at least one year of experience in mental health;
A minimum of a high school diploma or GED and five years of supervised clinical experience in mental health or human services; or
Any individual employed as an MHP prior to July 1, 2013 may continue to be designated as such unless their employment status changes.
