CentraCare Health System is seeking a highly skilled Nephrology Advanced Practice Provider to join their collaborative team.
Position Overview:
The Nephrology Advanced Practice Provider will practice in a state-of-the-art kidney program, providing patient care to outreach/dialysis sites and rounding at St. Cloud hospital. The ideal candidate will have experience in Nephrology and/or dialysis, with strong organizational management, leadership, and communication skills.
Responsibilities:
- Provide patient care to patients with End Stage Renal Disease/Dialysis, Chronic Kidney Disease, Acute Kidney Injuries, and Nephrology Related Diagnoses
- Treat and educate patients on anemia management, hypertension management, and chronic kidney disease education
- Collaborate with the Nephrology team to provide comprehensive care to patients
Requirements:
- Must be a Nurse Practitioner or Physician Assistant licensed in Minnesota
- Must have advanced practice prescriptive privileges
- Must have Basic Life Support (BLS) certification
- Valid Minnesota driver's license
