Clinical Nurse Practitioner or Physician Assistant

Matrix Medical Network is seeking a skilled Nurse Practitioner or Physician Assistant to join our team in providing high-quality care to our patients. As a member of our team, you will conduct comprehensive health assessments, provide medical history, diagnosis, and treatment, and collaborate with physicians, case managers, and other healthcare professionals to ensure the best possible outcomes for our patients.
About the Position:
As a Nurse Practitioner or Physician Assistant, you will have the opportunity to work in a variety of settings, including home visits, skilled facilities, and telehealth visits. You will be responsible for conducting adult and geriatric assessments, reviewing medical history, and providing health education to patients and their families. You will also collaborate with primary care physicians to ensure seamless care coordination.
Responsibilities:
- Conduct comprehensive health assessments, including medical history, diagnosis, and treatment
- Collaborate with physicians, case managers, and other healthcare professionals to ensure the best possible outcomes for patients
- Provide health education to patients and their families
- Collaborate with primary care physicians to ensure seamless care coordination
Requirements:
- Master's degree in nursing or a related field
- Current RN and NP or PA licensure in the state of practice
- Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
