Physical Therapist Home Health Specialist

CarePartners is seeking a skilled Physical Therapist to join our Home Health team in Franklin. Job Summary:
The Primary Physical Therapist in Home Health is responsible for managing an assigned caseload of patients, providing therapy and clinical case management to improve patient outcomes and manage resources effectively. This role requires a comprehensive assessment, development of a plan of care, and provision of treatment visits to facilitate patient engagement and achievement of goals.
Responsibilities:
- Conduct comprehensive assessments and evaluations to identify patient needs and develop individualized plans of care.
- Provide physical therapy services, including treatment visits, to patients in their homes.
- Collaborate with the care team to ensure seamless coordination of care and effective communication.
- Monitor patient progress, adjust plans of care as needed, and provide education to patients and caregivers.
- Document patient information, progress, and outcomes in accordance with regulatory requirements and CarePartners policies.
Requirements:
- Current North Carolina license to practice Physical Therapy.
- Driver's license required.
- BLS/CPR Certification.
- One to two years of related experience.
- Preferred experience in acute care settings, particularly in orthopedics, neurology, cardiopulmonary, and wound care.
