We are looking for a Physical Therapist for our Home Health Franklin Team to ensure high-quality, efficient patient care.
Job Summary and Qualifications
The Primary Physical Therapist in Home Health manages a designated caseload, providing therapy and clinical case management aimed at enhancing patient outcomes while managing resources effectively.
The role involves:
- Conducting comprehensive assessments and evaluations, developing and modifying care plans in collaboration with patients, caregivers, and physicians.
- Delivering treatment visits and facilitating discharge planning to engage patients and caregivers in achieving clinical goals and enhancing patient experiences.
- Coordinating care with Physical Therapy Assistants and other team members to ensure continuity of care.
Qualifications Required:
- Education: Bachelor of Science in Physical Therapy.
- License: Current North Carolina Physical Therapy license; valid Driver's license; BLS/CPR Certification.
- Experience: One to two years of relevant experience.
- Preferred Experience: Clinical affiliations in acute care settings, particularly in orthopedics, neurology, and cardiopulmonary care.
