About the Role:

CarePartners is seeking a skilled Physical Therapist to join our Home Health team in Franklin. As a Physical Therapist, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes. Our team is dedicated to delivering exceptional care and support to our patients, and we are looking for a compassionate and experienced Physical Therapist to join our ranks.

Responsibilities:

  • Conduct comprehensive assessments and evaluations of patients to identify their physical therapy needs
  • Develop and implement individualized plans of care to achieve patient goals and improve functional ability
  • Provide skilled physical therapy interventions, including exercises, modalities, and education, to promote patient recovery and independence
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication
  • Document patient progress and outcomes in accordance with regulatory requirements and CarePartners policies

Requirements:

  • Current North Carolina license to practice Physical Therapy
  • Driver's license required
  • BLS/CPR Certification
  • One to two years of related experience
  • Preferred experience in acute care settings, particularly in orthopedics, neurology, and cardiopulmonary rehabilitation
