Job Summary

We are seeking a highly skilled Licensed Practical Nurse to join our team at Sunnyside Operations LLC. As a Licensed Practical Nurse, you will play a vital role in providing high-quality patient care and contributing to the overall success of our skilled nursing facility.

Key Responsibilities
  • Develop and implement individualized care plans for patients, ensuring their physical, emotional, and social needs are met.
  • Administer medications and treatments as prescribed by physicians, and monitor patients' responses to care.
  • Conduct thorough assessments of patients' conditions, identifying areas for improvement and developing strategies to address them.
  • Collaborate with interdisciplinary teams to coordinate patient care, including physicians, therapists, and other healthcare professionals.
  • Provide education and support to patients and their families, promoting health literacy and self-advocacy.
  • Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that enhance patient care.
Requirements
  • Current, unencumbered LPN licensure in the state of Oregon.
  • Minimum of 1 year of experience in skilled nursing or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to lift, transfer, and position patients, as well as perform other physical demands of the job.
  • Basic computer skills and proficiency in electronic medical records systems.
