Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Avamere Health Services. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare team.
Key Responsibilities- Admission, Transfer, and Discharge - Complete accurate and timely record-keeping for patient admissions, transfers, and discharges.
- Shift Planning and Coordination - Collaborate with nursing staff and support personnel to plan and coordinate daily services, programs, and activities to ensure patient care needs are met.
- Medication Administration - Prepare and administer medications, reviewing medication cards for completeness and accuracy.
- Resident Care Planning - Review and revise patient care plans to ensure they align with patient goals, problems, and nursing needs.
- Leadership and Communication - Provide leadership and guidance to nursing personnel, making daily rounds to ensure staff is providing direct care in accordance with patient care plans and wishes.
- Rapport Building - Develop and maintain positive relationships with nursing staff, other department personnel, patients, family members, and other stakeholders involved in patient care.
- Facility Surveys - Participate in facility surveys by authorized government agencies.
- Nursing Degree - Possess a nursing degree from an accredited college or university.
- Licensure - Hold a current, unencumbered, active license to practice as an LPN in this state.
- Nursing Knowledge - Be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
- Charting and Documentation - Chart nurses' notes in an informative and descriptive manner that reflects patient care provided and patient response to care.
- Electronic Medical Records - Have experience with Electronic Medical Records and computer documentation systems.
- Confidentiality - Maintain confidentiality of all patient care information in accordance with HIPAA guidelines.
