Job Title: Clinical Coordinator RN

At Sparrow Health System, we are seeking a highly skilled and experienced Clinical Coordinator RN to join our team. This is a critical role that requires strong clinical judgment, leadership skills, and the ability to work effectively in a fast-paced environment.

Job Summary:

The Clinical Coordinator RN will be responsible for coordinating the clinical staffing and procedure needs of the Sparrow Medical Group. This includes supporting staff in assessing, planning, implementing, coordinating, monitoring, and evaluating care plans and services required to meet an individual's continuing health care needs in the most financially responsible manner.

Key Responsibilities:
  • Assist patients and/or families in recognizing and prioritizing problems that need to be addressed.
  • Provide support to help patients and/or families cope with the disruptions and distress associated with managing their illness and empowering them to regain control of their life, carry on normal roles and activities, and manage the emotional impact of their illness.
  • Problem solve with patients and their families to maximize their ability to follow preventive, remedial, and/or rehabilitative recommendations.
  • Facilitate follow-up with patients discharged from the hospital or Emergency Department (ED) as needed to assure patients understand self-care and treatment expectations and are seen in primary care offices on a timely basis.
  • Assist patient and family with access to appropriate resources and services based on a thorough knowledge of community resources and eligibility requirements.
  • Ensure the incoming patient admissions have been screened for insurance requirements and those necessary authorizations are sought prior to admission.
  • Ensure the patient type is entered correctly as Inpatient or Observation.
  • In conjunction with the Practice Supervisor, coordinate the nursing and medical assistant staffing needs of the practice, including assisting with the selection, training, and evaluation of personnel.
  • In conjunction with the Sparrow Quality Department and Supervisor, develop, implement, and monitor quality assurance standards for the Practice site per mandated guidelines.
  • Coordinate the development of educational materials and protocols for clinical practice, including providing counseling to patients and family members in relation to health and family matters, developmental needs, and crises.
  • Orient and oversee the continuing education of clinical staff, including providing guidance and assistance with the setting of goals and objectives for the nursing and medical assistant staff.
  • Conduct clinical-related in-services.
  • Under the direction of a physician, provide nursing care directly to clients within the limits of the RN license.
Requirements:
  • Minimum of five years acute care clinical experience as a registered nurse that required clinical judgment and critical experience.
  • Associates Degree in Nursing.
  • Current Michigan License to practice as a Registered Nurse.
  • Demonstrated ability to function in a team environment, including excellent written and verbal communication skills to effectively and appropriately communicate with clients, their families, physicians, and co-workers.
  • Ability to work in an EMR environment.
  • Ability to provide care for patients that is age-specific to meet their physical and safety needs, psychosocial concerns, and educational level.
  • Visual and auditory acuity necessary to monitor patients' condition and operate office equipment.
