Responsibilities
  • Provide intermittent skilled nursing services to patients in their homes, including assessment, evaluation, and treatment.
  • Develop and implement individualized patient care plans to address specific needs and goals.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Supervise and train home health aides to provide high-quality personal care services.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements
  • Current, unencumbered state professional nurse license.
  • Valid driver's license and good driving record.
  • Ability to make sound clinical judgments and assess patient needs.
  • Excellent communication and interpersonal skills.
  • Proficiency in electronic medical records and other clinical software.
