Job Title: Primary Care Physician

Abode Care Partners is seeking a highly skilled and experienced Primary Care Physician to join our team as a Clinical Physician Lead. In this full-time role, you will have the opportunity to provide high-quality, complex, and geriatric care to residents and patients across various settings, including skilled nursing facilities, assisted living, and private homes.

Responsibilities:
  • Provide high-quality, complex, and geriatric care to residents and patients across settings in partnership with non-physician practitioners and other interprofessional team members.
  • Collaborate with facility leadership teams, nurse practitioners/physician assistants, patients, and families to ensure high engagement and quality care.
  • Provide collaborating physician duties for nurse practitioners and physician assistants, supporting them in their clinical roles.
  • Audit medical orders and documentation for each provider monthly, based on state and regional regulatory frameworks.
  • Assume medical directorships for skilled and/or other senior living facilities, as needed.
  • Identify new growth opportunities with skilled facilities and/or other senior living facilities in the market/region.
  • Participate in company frontline/backup call processes for timely response to acute changes of condition and prevent avoidable hospital transfers.
  • Focus on quality outcomes, including care path compliance, productivity, prevention of avoidable transfers, and decrease in polypharmacy and costs of care.
Qualifications:
  • Graduate from an accredited school of medicine and a Doctor of Medicine.
  • Two or more years of administrative leadership experience in healthcare.
  • Clinical medical experience of seven years or more preferred.
  • Board certification in Family Medicine, Internal Medicine, or Geriatrics; qualification in Geriatrics preferred.
  • Practicing physician currently licensed under the provisions of the State Board of Medicine.
  • Active unrestricted licensure to practice in the state of employment.
  • Current Drug Enforcement Administration registration.
