Job Title: Community Health Worker

Meharry Medical College is seeking a highly motivated and compassionate Community Health Worker to join our team at Meharry Community Wellness Center.

Job Summary:

The Community Health Worker will play a vital role in providing comprehensive care to patients, focusing on health promotion, disease prevention, and health education. This individual will work closely with the multidisciplinary care team to ensure seamless patient navigation and access to essential services.

Key Responsibilities:
  • Develop and maintain strong relationships with patients, providing personalized support and guidance to promote healthy behaviors and adherence to treatment plans.
  • Conduct outreach services to individuals living with HIV, leveraging community partnerships to facilitate access to healthcare services and resources.
  • Collaborate with healthcare providers to ensure timely and effective patient navigation, including adherence counseling, harm reduction, and health promotion.
  • Collect and analyze data to inform program development and evaluate the effectiveness of services.
  • Attend mandatory meetings and participate in ongoing training and professional development to enhance skills and knowledge.
Requirements:
  • Bachelor's degree in a related field or equivalent experience.
  • Basic knowledge of HIV/AIDS and its impact on communities.
  • Excellent communication and interpersonal skills, with the ability to establish trust-based relationships with patients and community members.
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
  • Valid driver's license and willingness to travel to community locations.
Preferred Qualifications:
  • Familiarity with Nashville/Davidson County and the local healthcare landscape.
  • Experience working with Ryan White programs and Meharry Community Wellness Center.
