We are seeking a skilled RN Transitional Care Navigator to join our team at Ambercare. As a key member of our Hospice Liaison team, you will play a vital role in increasing public awareness of our services and driving admissions for patient placement.
Key Responsibilities- Collaborate with the Hospice Liaison team and Admissions team to develop and implement strategies for increasing referrals and admissions.
- Act as the first point of contact for hospitals, healthcare facilities, and other sources regarding referrals and admissions.
- Identify appropriate care and services for patients and families in accordance with attending physician and plan of care, as well as company policy and procedures.
- Bachelor's degree in marketing, business administration, or other related fields (or equivalent combination of experience and training).
- Licensed as a registered nurse in the state of practice.
- Ability to make independent decisions and deal tactfully with patients, families, and healthcare professionals.
- Valid driver's license and proof of insurance.
