**Job Summary**
We're seeking a skilled MDS and Care Plan Coordinator to join our team. As a key member of our interdisciplinary team, you'll be responsible for coordinating the completion of mandated OBRA and Medicare MDS assessments. Your expertise will contribute to the development, review, and revision of resident care plans. You'll serve as a resource for all disciplines related to the RAI process and MDS assessments.
**Requirements**
* Must be a Registered Nurse in good standing and licensed with the applicable State Board of Nursing
* Previous MDS 3.0 experience preferred
* Excellent communication skills in both written and oral communication
* Knowledge of computer skills
* Full understanding of State and Federal RAI regulations with the ability to communicate and comprehend the MDS process
* Knowledgeable about reimbursement rules and procedures
* CEU's as required by State
