POSITION OVERVIEW
The RN Case Manager acts as a vital advocate for patients, assisting them and their families in navigating their healthcare journeys. This role involves developing comprehensive care plans, ensuring efficient resource utilization, and creating safe discharge strategies.
KEY RESPONSIBILITIES
Responsibilities include but are not limited to:
- Assessing patients for clinical, psychosocial, and financial factors impacting their care.
- Collaborating with patients and families to establish care goals that align with their needs.
- Coordinating services among various healthcare providers and community resources.
- Providing patients and families with essential tools and information to make informed healthcare decisions.
- Creating care plans that are tailored to the patient's specific needs and preferences.
- Facilitating communication during transitions of care to ensure all parties are informed and engaged.
- Identifying community resources to address gaps in care and advocating for patients.
- Maintaining compliance with relevant healthcare regulations and standards.
- Participating in multidisciplinary rounds to ensure appropriate patient care and resource allocation.
- Tracking and addressing any delays in care to promote efficient healthcare delivery.
- Educating patients and families about the financial implications of their care options.
- Engaging in performance improvement initiatives to enhance care delivery.
- Collaborating with the utilization review team to meet departmental objectives.
- Recognizing and referring situations that require quality or risk management intervention.
QUALIFICATIONS
To be considered for this role, candidates should possess:
- An Associate Degree in Nursing (BSN preferred).
- A minimum of one year of experience in discharge planning, case management, or as a Registered Nurse in a healthcare setting.
- A valid Registered Nurse (RN) license in the state of Florida.
- Basic Life Support (BLS) certification from a recognized provider.
- Preferred certification in Case Management.
