Mass General Brigham is seeking a skilled Registered Nurse (RN) to fill a Case Management position in Belmont, Massachusetts.
Job Summary:We are looking for a dedicated and compassionate Registered Nurse to join our team as a Case Manager. The successful candidate will be responsible for coordinating patient care, developing and implementing individualized care plans, and collaborating with healthcare teams to ensure seamless transitions and optimal patient outcomes.
Key Responsibilities:- Assess patient needs and develop individualized care plans in collaboration with healthcare teams.
- Coordinate patient care, including arranging for necessary services and resources.
- Communicate effectively with patients, families, and healthcare teams to ensure timely and informed decision-making.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Collaborate with healthcare teams to ensure seamless transitions and continuity of care.
- Current, unrestricted license to practice as a Registered Nurse in Massachusetts.
- Minimum 3-5 years of experience in case management or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a focus on collaboration and coordination.
- Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
- Master's degree in nursing or a related field.
- Case management certification.
- Experience working in a hospital or healthcare setting.
