Hemodialysis Nurse

We are seeking a highly skilled and compassionate Hemodialysis Nurse to join our team at Yale New Haven Health. As a key member of our dialysis team, you will be responsible for providing high-quality patient care and education to individuals undergoing hemodialysis treatment.
Key Responsibilities- Assess and plan individual patient care on a nursing unit/department according to unit policies and procedures.
- Develop, communicate, and document a plan of care related to the patient's acute dialysis schedule and response to treatment.
- Implement and coordinate the plan of care in collaboration with the nephrologist on call.
- Evaluate the effectiveness of the dialysis treatment and patient response, providing educational opportunities and documenting outcomes.
- Plan for education, assessing individual learning needs and readiness of the patient and family, and providing educational opportunities and documenting outcomes.
- Function as a preceptor to new staff and as a charge nurse.
- Meet performance expectations for customer service, teamwork, resource utilization, and staff and self-development as outlined in performance review.
- Graduate of an accredited school of nursing; BSN preferred.
- Proficient in the use of computer software, including electronic patient records.
- Ability to use the internet for literature searches regarding patient care.
- Med-surg experience preferred; previous experience with renal patients required.
- Valid State of Connecticut Registered Nurse license.
- BLS required and ACLS within 6 months.
