Job Summary
In this role, you will play a crucial part in recommending and facilitating access to a variety of support services aimed at helping clients meet their personal objectives. This involves collaborating with healthcare professionals, advocating for clients, and ensuring they receive comprehensive care tailored to their needs.

Key Responsibilities
  • Perform assessments, deliver therapeutic interventions, create individualized care plans, provide crisis support, and assist clients with essential needs.
  • Work alongside the Peer Support Specialist to offer field-based services effectively.
  • Ensure precise documentation and adhere to service requirements for billing purposes.
  • Assist in client placements and arrange necessary transportation.
  • Engage in on-call responsibilities and participate in program meetings.
  • Act as Care Coordinator for designated clients and collaborate with community service providers.
Qualifications
  • Master's Degree in Social Work, Psychology, Counseling, or a related discipline.
  • Registered as an Associate/Intern in the State of California.
  • Experience in mental health and/or substance use disorder services is preferred.
  • A valid California driver's license and auto insurance are required.
  • Cultural competency is essential; bilingual candidates are encouraged to apply.
  • Proficiency in Microsoft Office and familiarity with EHR software is beneficial.
