Amergis is seeking a skilled RN Care Coordinator to support the transition of care from facility-based care to community-based care.

Key Responsibilities:
  • Reach out to facility contacts to obtain discharge information and associated plans for patients to support their post-transition care.
  • Develop a plan with the patient to meet the identified post-discharge needs, which may include home visits, accompanying patients to their first visits, or scheduled phone calls.
  • Interface with care teams to ensure the application of care coordination protocols for screening assessment and care planning.
  • Report any incidents of suspected abuse, neglect, or exploitation to the supervisor immediately upon discovery.
  • Report feedback to medical home teams according to department policy and procedure.
Requirements:
  • Bachelor's Degree of Science in Nursing (BSN) from an accredited college or university.
  • Licensed as a Registered Professional Nurse in the State of Illinois.
  • 2 years of recent work experience with responsibilities for care coordination across multiple healthcare settings and providers.
  • 2 years of recent experience using electronic medical records, care management systems, or scheduling software.
  • 1 year of recent experience in utilization management or case management.
  • Valid Driver's license and mandatory vehicle insurance or other dependable means of transportation.
