Amergis is seeking a Registered Nurse to provide care coordination services in Chicago, IL.

Job Responsibilities:
  • Collaborate with patients, medical home care teams, and other stakeholders to develop and implement care plans that address patient needs and barriers.
  • Assist patients and their support persons in accessing covered healthcare services and resources.
  • Support the implementation of new workflows to optimize healthcare utilization and cost alignment with departmental policies and procedures.
  • Manage patient care according to care coordination protocols, policies, and procedures.
  • Identify patient care issues, develop approaches to resolve them, and escalate to the Manager of Complex Care Coordination as needed.
  • Interface with care teams to ensure the application of care coordination protocols for screening, assessment, and care planning.
  • Travel to patients' homes or sites of care as required.
Requirements:
  • Bachelor's Degree of Science in Nursing (BSN) from an accredited college or university.
  • Licensed as a Registered Professional Nurse in the State of Illinois.
  • 1 year of recent work experience in care coordination across multiple healthcare settings and providers.
  • 1 year of recent experience in case management.
  • 1 year of recent experience in ambulatory nursing, preferably in the ED setting.
  • Valid Driver's license and mandatory vehicle insurance or other dependable means of transportation.
