Job Details

  • The successful candidate will be field-based, responsible for providing care management services to assist with the coordination of care, including assessing healthcare needs, referring to health plan providers, educating clients on benefits, and connecting them to community resources addressing social determinants of health.
  • The caseload comprises 100 members.
  • This position requires documentation in an electronic health record, including outreach notes, assessments, and care plans for each member.

Key Responsibilities:

As a Resource Management Professional, your primary duties will include:

  1. Providing case management services to 100 members.
  2. Documenting all interactions and care plans in an electronic health record.
  3. Collaborating with healthcare providers to coordinate care and address client needs.
  4. Connecting clients to local resources for employment, housing, independent living, justice, foster care, and other essential services.

Requirements and Qualifications

  • Bachelor's degree required.
  • Prior experience in child welfare or pediatric case management is highly desirable.
  • Ability to work independently and as part of a team.
