About the Role:

As a Nurse Practitioner, you will be responsible for conducting in-home and telehealth visits, providing primary medical care and palliative care for patients with chronic and acute illnesses.


Key Responsibilities:
  • Conduct in-home and telehealth visits for patients in our service area
  • Provide primary medical care and palliative care for chronic and acute illnesses
  • Prescribe and manage medications and other therapies
  • Interpret laboratory and test results
  • Provide physical exams and diagnose and treat acute and chronic diseases

Requirements:

We are seeking a candidate with a current Louisiana license, excellent communication skills, and problem-solving skills. Hospice and/or Home Health experience is required.
