About the Role:The Case Manager will be responsible for coordinating patient care across the continuum, from hospital admission to discharge. This will involve working closely with healthcare teams, patients, and families to ensure that patients receive the best possible care and support.
Key Responsibilities:- Coordinate patient care across the continuum, from hospital admission to discharge.
- Work closely with healthcare teams, patients, and families to ensure that patients receive the best possible care and support.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with healthcare teams to ensure that patients receive timely and effective care.
- Provide education and support to patients and families to promote health and well-being.
- Monitor and evaluate patient outcomes to ensure that care is effective and efficient.
- Graduate of an approved school of nursing, BSN required. MSN preferred.
- Current license to practice as a registered nurse in the State of Massachusetts.
- Case Manager Certification preferred.
- 5+ years of Case Management experience at an acute care hospital.
- Must have extensive knowledge of case management.
- Experience designing and/or utilizing innovative approaches to optimizing capacity, inflow/outflow, and saving bed-days preferred.
- Experience with Repatriation preferred.
