Job Overview:
We are looking for a dedicated Care Coordinator to become a vital part of our team.
In this position, you will collaborate closely with patients to assess their healthcare needs, formulate the most effective treatment strategies, and manage their care plans, ensuring that each individual receives exceptional, personalized attention.
The ideal candidate is empathetic, dependable, trustworthy, and well-versed in healthcare protocols.Key Responsibilities:
Engage with physicians, patients, families, and healthcare personnel
Evaluate Care Plans to ensure compliance; if necessary, provide updates and training
Design tailored care plans
Accompany clients to medical appointments
Establish objectives and track progress towards those objectives
Recruit and mentor staff
Develop work schedules
Oversee a caseload of 30-40 clients
Required Qualifications:
Prior experience as a Care Coordinator or in a related role is preferred
Certification as a medical assistant is a plus
Strong organizational and problem-solving abilities
Capability to manage various projects or tasks and prioritize effectively
Aptitude for thriving in fast-paced environments and making prompt, sound decisions
Exceptional interpersonal skills and a high degree of empathy
Strong verbal and written communication skills
