Job Summary

The Brevard Health Alliance Inc is seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team in Pediatrics. As a Primary Care LPN, you will play a vital role in providing high-quality, evidence-based care to culturally diverse populations with acute and/or chronic illness in our outpatient care setting.

Key Responsibilities
  • Collect data, document using appropriate forms and electronic health records, follow policies, protocols, and guidelines, adhere to policies, and evaluate resources for each shift.
  • Implement patient activities towards health promotion and disease prevention, assisting the patient in attainment of optimal levels of function through direct care and non-face-to-face communication.
  • Perform routine administrative and clinical duties to assist the nurse, MD/DO/APRN/ or PA as part of the medical home healthcare team.
  • Maintain daily patient flow to ensure patients will be seen as soon as possible using all available resources in EMR.
  • Check patients in for appointments and gather vital signs.
  • Orders labs and diagnostic tests appropriate for the LPN scope of practice following policy/protocol.
  • Answer desktop encounters and messages within 24-48 hrs.
  • Call back patients within 24-48 hrs.
  • Evaluate and work to close clinical gaps as indicated.
  • Perform huddle with healthcare team.
  • Participate in reaching annual Patient Satisfaction goals.
  • Perform pre-appointment phone calls and chart preparation.
  • Participate in departmental continues quality improvement efforts.
  • Maintain clinic nurse schedule, to ensure timeliness, flow, and appropriateness as needed.
  • Assists in the Management of clinic telephone calls.
  • Able to perform and carry out projects as delegated by BHA DOCO and/or Managers.
Requirements
  • License from accredited School of Nursing.
  • Clear and active Florida or compact Nursing License.
  • Minimum of four (4) years nursing experience with two (2) years recent experience in an outpatient/ambulatory care setting preferred.
  • Current Basic Life Support (BLS)/CPR certification from the American Heart Association or the American Red Cross.
  • Proficient in Electronic Medical Records (EMR).
  • Demonstrates strong oral and written communication skills.
