Job Description

Aveanna Healthcare is a leading provider of home health services to medically fragile children and adults. We are seeking a skilled and compassionate Private Duty Nurse LVN to join our team.

Job Summary:

  • Provide high-quality, patient-centered care to individuals in their homes.
  • Assess patient needs and develop individualized care plans to meet those needs.
  • Implement and evaluate the effectiveness of care plans in collaboration with healthcare teams.
  • Document patient progress and communicate with healthcare teams to ensure seamless care.

Responsibilities:

  • Implement physicians' prescribed plans of care.
  • Conduct patient assessments and coordinate care with healthcare teams.
  • Administer prescribed medication, treatments, and therapies.
  • Document care provided in a timely and accurate manner.
  • Provide education and support to patients and families to promote health and well-being.

Requirements:

  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.

Preferred Qualifications:

  • Six months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
