We are seeking a skilled and compassionate Private Duty Nurse LVN to join our team at Aveanna Healthcare. As a Private Duty Nurse, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Job Summary

The Licensed Practical Nurse (LPN/LVN) is responsible for delivering and coordinating quality patient care in compliance with physician orders. This includes continuously observing and assessing patient condition and care needs, documenting all activities and assessments, and participating in the development and implementation of the nursing care plan.

Responsibilities
  • Implementation of physicians' prescribed plan of care
  • Patient assessments and coordination of care
  • Administration of prescribed medication, treatments, and therapies
  • Compliant, timely documentation of care provided in system of record
  • Health promotion, teaching, and training of family members
  • Accepting and writing verbal orders from physicians as required and for obtaining co-signature of Case Manager on all verbal orders
  • Follow up with physician, client, family members, staff, and Case Manager on client problems, concerns, and/or changes in client condition
Requirements
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
