Job Title: Private Duty Nurse LVN

Aveanna Healthcare is seeking a skilled and compassionate Private Duty Nurse LVN to join our team. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes, assisting them with transitioning from hospital to home and providing the continuous care needed to remain at home with their loved ones.

Responsibilities:
  • Implement and manage patient care plans in collaboration with physicians and healthcare teams.
  • Conduct thorough patient assessments and coordinate care to ensure optimal outcomes.
  • Administer prescribed medications, treatments, and therapies as ordered by physicians.
  • Document patient care and progress in a timely and accurate manner.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Collaborate with healthcare teams to ensure seamless care transitions and address patient concerns.
Requirements:
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification from American Red Cross, American Heart Association, or BLS.
  • TB skin test (current within last 12 months).
  • One year of experience as a Licensed Nurse in a clinical care setting, with a focus on pediatrics or home health.
