We are seeking a highly skilled Behavioral Health Care Coordinator to join our team at Adapt Integrated Health Care.
The successful candidate will be responsible for promoting a healthy and supportive environment by addressing barriers to treatment and providing warm handoffs between departments and/or outside providers when necessary.
Key Responsibilities- Coordinate Services: Work with individuals diagnosed with mental illness, substance use disorders, or primary care needs to promote recovery, independence, and successful community living.
- Provide Advocacy and Linkage: Assist individuals in navigating between external systems (DHS, CCO, etc.) and Adapt services, while promoting advocacy and linkage in planning, monitoring, and coordination.
- Problem-Solving and Resource Development: Assist individuals in problem-solving to reduce barriers to recovery and support the development of resources within the community, while coordinating services with other community and treatment providers.
- Education and Experience: Associates Degree or High School Diploma and preferred two years experience in a social service and/or mental health setting, or a satisfactory equivalent combination of education, experience, and training.
- Valid Oregon Driver's License: Must possess a valid Oregon driver's license at the time of appointment.
