Family Nurse Practitioner

Coast Community Health Center is a non-profit rural health center dedicated to providing high-quality, affordable primary health care services to the Bandon community and surrounding areas.
Our health center was established in 2009 and has since expanded to include multiple clinics and school-based health centers. We are committed to making medical services accessible to all, regardless of income or social status.
The RoleThis position is a fundamental part of our primary care team, providing services to a diverse population, including newborns, infants, children, adolescents, adults, pregnant and postpartum women, and older adults.
As a Family Nurse Practitioner, you will work collaboratively with our care team to ensure the highest level of patient care, focusing on assessment, diagnosis, and management of health and illness, as well as prevention of disease and patient wellness programs.
Responsibilities- Obtain and document relevant health histories for patients of all ages;
- Perform and document comprehensive or symptom-focused physical examinations on patients of all ages, following medical record documentation and coding guidelines;
- Identify health and psychosocial risk factors of patients of all ages;
- Plan interventions to promote health with families at risk;
- Identify signs and symptoms of acute and chronic physical and mental illnesses across the life span;
- Order, perform, and interpret age-, gender-, and condition-specific diagnostic tests and screening procedures;
- Formulate comprehensive differential diagnoses;
- Provide health protection, health promotion, and disease prevention interventions/treatment strategies to improve or maintain optimum health;
- Treat common acute and chronic physical illnesses and common injuries in people of all ages;
- Prescribe medications with knowledge of altered pharmacodynamics and pharmacokinetics with special populations;
- Adapt care to meet the complex needs of older adults arising from age change and multiple system disease;
- Assist individuals and families in developing coping systems and lifestyle adaptations;
- Coordinate with nurses and/or medical assistants to make appropriate referrals and coordinate transitions to other health care professionals, agencies, and community resources;
- Provide anticipatory guidance, teaching, counseling, and education for the patient and family based on assessed need and required treatment plan;
- Participate in community and population-focused programs that promote health and prevent or reduce risk of illness;
- Collaborate as a member of the interdisciplinary team;
- Provide consultation to health care providers;
- Use the therapeutic relationship to promote positive clinical outcomes;
- Identify and maintain professional boundaries to preserve the integrity of the therapeutic process;
- Seek consultation actively with the Chief Medical Officer, physicians, and professional colleagues;
- Refer patients requiring more specialized or complex treatment to the most appropriate resource.
- Participate in staff conferences and serve on committees under the direction of the Medical Director;
- Complete medical records and reports in a timely manner;
- Coordinate with support staff to ensure continuity of care;
- Encourage professionals to create health and wellness programs and services for the betterment of patients and the community, teaching of classes or support groups, and medical educational outreach to the school districts, community, geriatric community of patients, and general education to patients and community groups supported by the health center.
- Bachelor's and master's degree and/or PhD in Nursing from an accredited college of nursing;
- Completion of an accredited FNP program;
- Maintains Board Certification (FNP-BC);
- Oregon active licensures with no adverse action;
- Oregon prescriptive authority;
- A minimum of five years-experience in a primary care setting;
- Experience in a rural setting is preferable.
- Ability to understand and operate necessary computer equipment and software programs;
- Ability to speak a foreign language helpful, though not necessary;
- Must be self-motivated.
Frequent contact with employees, the public, and patients requiring interviewing, good listening, and communication skills.
SupervisoryModest direct and indirect supervision of other non-professional clinical staff.
