As a Patient Navigator, you will play a vital role in providing culturally sensitive, clinically competent, and respectful services to high-risk birthing families.

About the Role
We are looking for a skilled Registered Nurse with prior experience working with women, infants, and children. The successful candidate will have a positive attitude, excellent listening skills, and the ability to maintain detailed records. They will be responsible for providing nursing assessments, interventions, screenings, referrals, service coordination, and diagnosis-specific education to improve the health, social, educational, and developmental needs of high-risk dyads throughout pregnancy and the first one year after birth.

Key Responsibilities
- Provide home visits monthly for the duration of pregnancy and at least the first three months after birth.
- Offer opportunities for visits outside of the standard schedule to accommodate working families and/or urgent/unexpected needs.
- Conduct comprehensive needs assessments and care plans, including family desired goals for health and wellness.
- Utilize motivational interviewing style of communication to discuss diagnoses or risk-specific education in alignment with family desired goals.
- Enhance health and well-being of high-risk birthing families by promoting maternal mental health concerns, substance use identification and treatment, and safe infant sleep environments.

Requirements
- Registered Nurse required.
- Prior experience working with women, infants, and children preferred.
- Positive attitude, empathy, and effective communication skills.
- Detail-oriented, critical thinking, and organized with ability to maintain detailed records.
